I have also experienced the same issue before. I removed my phone number as an alias and as a result, I can't re-add it to my account even to this day. What I did was add my other phone number which is not associated with any Microsoft accounts and that seemed to work. There could be an issue with the account, phone number, or the server.

Choose the call option: During the verification process, you have the choice between receiving a text message or a phone call for the verification code. If the text message option is not working, select the call option and enter your home phone number.

if you have been accepted into the rewards program then you will get a emial but it may take a while for the email to come though. mine took longer then 36 hours to come though so i allowed at least 7 days for it to come though also check your spam folder
on your email it may of gone in there.

When was you accepted? Just a wait a bit in it should come. Mine took longer than 36 hours for me to get the email. And go to
http://rewards.xbox.com/ and push sign in. If your stuff comes up than that's how you know if your in the rewards program.
