Xbox controller disconnected and wont reconnect

I recently swapped my two controllers faceplates. since I did it worked once, when putting my console to sleep (not fully powered down) my controller wont connect. when i turn it on it immediatly goes into pairing (not unable to connect to anything) mode. When i pair the controller it rumbles twice then shuts off. Any help?

Hello Ujustlost210, when it says that you changed the faceplate of the control you opened it, if yes check if no part suffered damage when closing, have also tried to update the control either on Xbox or PC by the Xbox accessories app.

Same problem as mudbiscuit and KSAnnihilation, My controller disconnects during forza horizon 2 and wont reconnect until i power cycle the console

Perform a hard reset EXACTLY like this.

Hold the power button on the console for 8 seconds or until the power shuts down completely. Unplug the power cord from the back of the console for 5 minutes. Plug the power cord back in and restart the console.

I will try this tonight and let you know.

is anyone having issues where the controllers are constantly disconnecting from the xbox one and then won't reconnect? We got this same console and the controllers disconnect every few days and to get them to reconnect we have to unplug the xbox for 20
seconds then reconnect it. using the connect buttons on the controller/console does nothing for some reason (no issues like this on the regular xbox one we have). very disappointing.
