Why do I have to repurchase already bought games?

This may mean that it's purchased under a different Microsoft account that you own. Please sign in to https://account.microsoft.com/billing/orders and then locate the transaction for the game, it should be stated as "completed"

Once confirmed that it is purchased under the correct account remove and re-add your account to re-sync the license:

" REMOVE ACCOUNT​​​

2. Select System > Settings > Account > Remove accounts. ​​​

3. Select the account that you want to remove, and then select Remove to confirm. ​​​

4. When you're finished, select Close. ​​​

2. Select your gamerpic in the upper-left corner of the screen, move all the way down and select Add new. ​​​

3. Enter the Email address and the password.​​​

4. Read the Microsoft Service Agreement and Privacy Statement, and then select I Accept. ​​​

5. Follow the steps to configure your Sign-In & Security preferences to add your Microsoft account to your Xbox console.​​​"

Another thing I noticed was on the marketplace it lists Fallout 3 as 'not purchased', but it's in my download history... why is trying to get me to repurchase a game I've already bought?

Having achievements for a game doesn't necessarily mean that you've purchased it on that account. You could have played it at a friends house, another family member on your console could have purchased it, you could have purchased it on another account,
etc.

You're saying you've looked at your download history,and the game is not listed there. I'm not asking you to go to the game and try to purchase it, I'm asking you to look at your download history, two very different things. If you've confirmed by looking at
your download history that it doesn't show up there then you will need to
contact phone support if you're sure that you purchased it on this account.

Please go to your Download history and check for the game there. Please remember that you can only download the game from the account you purchased it on.

You can check your download history by going to your Dashboard > Settings > Account > Download History.
