Ive resolved it. my problem was the browser (safari on my mac). I used the code via Chrome on my android tablet and it worked first time.

Try using the code for Digital Combo on 360 or Xbox.com after redeeming code go to Ghosts on Xbox One and download.

From a review on Amazon Digital Combo version;

You simply boot up your xbox 360. Go to redeem a code and then download the game. Once that is done. Log onto your xbox one. And go to games and then obviously cod ghosts. And then it says install free. You need to make sure that you use the same gamer tag.

Got a decent package offer from game. 500gb hard drive with headset, joypad and COD GHOSTS (disc) and COD BLOPS 2 (Redeem code) for £189.99 brand new.

I also have this exact issue, both with the FreeFall DLC that came with the COD Ghosts Premium Bundle and three Day One cars for Forza 5.

Has anyone found a fix for this or heard from Acttivision? I will try to see if the codes are Redeem Codes are "US" as suggested by Twistedflax below
