From what you have advised, as there appeared to be nudity in your picture, if reported, it will have failed the review resulting in the image being changed. I personally had this where I had a picture of one of my friends and it looks like he had a red armband on, similar to that from WW2, safe to say i change that one as soon as it was flagged!

It seems that because of the epidemic a lot of services has been slow down like internet from various companies as well as some features on Xbox because of the traffic so some things might be disabled temporarily like profile pictures and various other
things

• There are 2 options when you edit your profile picture
  
  

• • Gamer picture - this is the picture others see when viewing your profile
    
  • Personal picture - this is the picture your friends and you see when viewing your profile
    

Are those both set to the Bills?

If so, try these steps:

1. Clear your system cache
   
2. Restart your console
3. Download your profile
