In order to ask for a refund please use your child account and access to the order history

Then, proceed with the refund form here:

In case your refund gets denied, you can actually file an appeal for the results of the refund.

All you need to do is to reply to the email that was sent to you by the refund team, you can reply with a detailed explanation so they can review your request again and grant you a possible exception.

Still no refund, so Microsoft have basically stolen my money. May raise with credit card company seen as no response from Microsoft

When has it ever been stolen? That's not a thing. It's a rumour. If your credit card was truly stolen it's by someone you know that has access to the account.
