the 4 digit code for the family password can only be reset by calling support. There is nothing we on the forums can do about it.

If it's a 4 digit code to get into your account then you need to delete your profile from the console.

I would suggest trying to redeem the code again and if that doesn't work the only real option is to contact Xbox support by phone. I know you said that you couldn't do that but they are the only team who could look up your account and pre -paid code and
see what is going on.

This may be a silly question Badger but is it 12 months from the first change or from the last? I'd say the latter but you never know how the minds of company executives work.
