So I'm running my Xbox of a wifi extender. It keeps changing ports and randomly gets packet...

The usual troubleshooting steps.

1. There must be some default setting that makes the extender randomly change ports as it transfers the packets from the "extender" network to the main "router" network. May be you can get the extender to use a specific port and stop the random change

1. Just to be on the safer, connect directly the main router and see if the issue is not happening. At least you can isolate the problem is the extender and not the main wifi. You can never be too careful with these things
1. Worst case scenario, your only solution might be to simply draw a long LAN cable directly from the main router (this is what I did in my house) to the Xbox. It makes things look ugly (long wires on walls) but if you get creative, it's the best solution. gaming on WiFi is always a drag.

Its also possible to get a wifi extender, that has network ports on it, and connect the extender to your router, then the extender via hardwire to your xbox.

So what can I do to fix my problem of my ports randomly changing and if possible, keep my nat type open?

It keeps lagging me out of games randomly, Madden 20 more so. I've tried both wired, and wireless. My download speed is 212. Mb, upload runs anywhere from 10 to 12 Mb packet loss 0, and latency 40-60%. In which those numbers drop when I get kicked, except
packet loss rises. I know that 4 is the standard im curious if IPv6 would fix the lag and or me getting booted.
