Rise of the Tomb Raider exclusive add-on still missing

I still haven't received the Tactical Survival Kit add-on.

Xbox 360 Rise of the Tomb Raider Achievement missing. For my next trick.

 

Item in question: Tactical Survival Kit add-on that is supposed to come with the Rise of the Tomb Raider console bundle.

Date of purchase: November 9th 2015.

I got the console bundle. I actually provided xbox support with details of the purchase and the console's serial number to help solve the problem of the missing add-on. I understood from the emails that the problem was identified and people started getting
their codes via the xbox messaging system, but unfortunately I haven't received one yet myself.

I could provide the documents (again), but I'd rather not upload them online somewhere because they contain my personal information, and private messages don't seem to support attachments. I sent them to an xbox support agent via email about a month ago.

The documents were receipts and proof of purchase basically, not sure if that's the info you're after
