Are you trying to request a controller replacement through the following support tool?

https://support.xbox.com/xbox-one/controllers/controller-warranty. If so I would also try using the form using different browsers and devices.

If the issue continues on other devices and web browsers, then try setting up a replacement from here https://support.microsoft.com/devices, however in order to do
so you need to own an Xbox One console to register and then create an accessory order.

If you do not own an Xbox One console or need further assistance, then I suggest contacting Xbox Customer Support using the following steps:

1. Contact us here
2. Select Xbox One
3. Select See other options
4. Select Payment & billing under Account management
5. Select show contact options
6. Select "Request a call from Support" if available within hours of operation or there should be a number for you to call and hours of operation for your region if a callback option is not available

You will still have to ship the control as requested, depending on the problem they will either ship a new control to you or repair it if possible.

If this happens, try and submit a repair request form in Support at Xbox.com or just get a new controller.
