Redemption code redeemed by someone else

I did not receive an email for redeeming the code. So is it possible the code is no good or someone else redeemed it?

Was it a Canada redeemable code or a US code and was it downloaded on your console or someone else's?

There's not really anything else anyone here can do if the form won't fill out. The only other thing I can do is suggest checking your order history on the redemption page of the Rewards site to see if it shows any of your redemptions. If it does, it'll either list a code there to redeem or give the option to resend them. Keep in mind, it can take up to 24 hours to receive anything you redeem from Rewards after you redeem it.

Any included games will be delivered via a 5x5 redemption code. Some redemption codes include all the content. Have you redeemed the code to see what all it included yet?
