You would have to try contacting support.

Try performing a hard reset EXACTLY like this. Hold the power button on the console for 8 seconds or until the power shuts down completely.
Unplug the power cord from the back of the console for 5 minutes.
Plug the power cord back in and restart the console.

If that doesn't help you should set it up for a repair.

However, with everything you have tried you will probably have to set it up for a repair. It should still be under warranty. You should contact
Xbox Support directly should you need to have it repaired.

You should receive an email with what you are supposed to send in.

Normally all you would need to send in is the console itself no other accessories or power cords or anything like that.

Also DO NOT send it in the original retail box you WILL NOT get that original box back.

The ONLY time you would need to send in any thing other than the console is if you were sending in the controller for repair.

And then if it is one of the elite controllers you would NOT send in any of the extra thumbsticks or back sticks with those.

If there is a game that is stuck in the console you would want to put that in a special notes section on the order itself so that they know with what game it is that is stuck in the console.
