6. Your name and the email address associated with your account will be auto-filled after you sign in. If you have a preferred contact email address, it can be updated on the form.

The only problem is: where?

I cannot find the proper way to ask for a refund. If I log onto the Xbox page support there's no way to contact anyone, the only thing you can do is to write on the Forum.
