try turning off the vibration and if that still doesn't work try doing what did and buy a charge and play kit for the 360 controller.

Firstly is your headset wired or wireless, if it is wireless you will need to reconnect it to the xbox after a reset as the xbox has now removed anything that was connected to it, if it is a wired headset then maybe try disconnecting the controller and reconnecting
it again to see if that helps.

Thanks for keeping us updated. Make sure the console is plugged directly into a wall outlet. Plugging into an ext. cord or surge protector can cause a lot of different issues if the console is not getting the power it needs.

Thank you. It is connected directly to the wall outlet and is still experiencing the malfunction.
