purchased new xbox with the promo to choose one of four games, the only code included in the box was a 14 day free trial for xbox live, and no code to redeem one of the games, what should my next step in my situation be?

There should be a code included inside the box that had the console. Then you simply redeem the code from the console and the download of the game should start.

Hi, there may be a code card inside the game box, you would need to redeem the code in that case, or the game once installed may have the content included already.

You would need to try either filling out the form at the bottom of the offers' FAQ -

You should have gotten a piece of paper inside the box with a code on it. You need to redeem that code on your xbox or at xbox.com
