It sounds like you added the gold after you purchased Ultimate. When you add gold or game pass while you have Ultimate, it'll automatically convert them into Ultimate since Ultimate already includes those (it warns you of that when you redeem the code).
The codes are converted based on price rather than time. 12 months of Gold is equal to 4 months of Ultimate. By completing the purchased, you agree to those terms.

Now that the Ultimate subscription ended, it sounds like auto-renewal was left turned on. So they have started a new subscription. If they haven't been able to charge you for it, your subscription would be suspended until you do pay it off. You can try contacting

My Xbox Live Gold subscription was cancelled out of nowhere without my knowledge. I renewed the subscription a few months ago with a 12 month subscription. I heard it might be because my Game Pass subscription ran out, but why would that affect my Gold?? Please explain.

If you had an active Ultimate Game Pass subscription, then any Live Gold added to your account was converted to UGP at a price ratio...so, in the case of a 12 month live Gold code, it converted into 4 months Ultimate Game Pass as they are the same price.

Sounds like that is why it has run out.
