My gamer tag got changed?

Yesterday I had used my Xbox One in the afternoon, left, and when I got home and turned it back on it took me through all of the initial startup steps (power mode, account, privacy, etc.). So I think maybe it just crashed and I enter
my account information again. When it takes me to the home menu my gamer tag is one of the randomly generated ones (not my actual gamer tag), my gamer score has been reset to 0, and my 6 or 7 months of Xbox Live Gold are gone. I tried removing the "dummy"
account and re-entered my account information, but it logged me back in to the same randomly generated gamer tag account. It's doing the same thing on here as well. How do I get my old account/gamer tag back? I lost all of my achievements, rewards, etc. Thanks.

Hey did you ever get a resolution to this issue? I am having the same problem. I haven't been on for a couple of months but i know my xbox live membership is still valid, i think i have a couple more months on it. However all of my achievements and ranks
are now gone, like on Halo 5 which is the only game i really played. I logged back on today and played a game just to see if my live was working and it is but now i am a rank 2 before i was in the 90's. I need help.

Why did my gamer-tag change? It really didn't offend nobody ( BIgBOyLuvMiLPhz )it was a funny gamer-tag to make people laugh and this was my first time actually getting reported. My friends gamer-tag were funny but when they got reported they had to change
there gamer-tag to something else or they couldn't play live at all and you gave them a (FREE GAMER-TAG CHANGE), how come I cant get that opportunity to change my gamer-tag?

You were given a forced gamertag change by the enforcement team because they felt it violated the code of conduct.. For information about forced gamertag changes, you would need to go to
http://enforcement.xbox.com

This one will most likely also get changed.

It is because the original Gamertag violates the Code of Conduct in regards to offensive Gamertags. The original Gamertag contains a reference to a member of make anatomy, except it is misspelled. It clearly sounds like the actual body part you were referencing.
This is why your Gamertag was changed. Your current Gamertag is also in violation as it still refers to a certain male body part, albeit the spelling looks different, it still sounds like the body part in question.
