MY disk got chiped in the xbox one

Recently I was playing Black Ops 3 and my game suddenly stopped working, at first I though the game had just crashed, but I keep trying but the game would not open. I took the disk out and it was chipped, what should I do.

Microsoft has a program for Disc replacement on Microsoft published games, but since this is an Activision title, I would recommend looking into the program that they offer.

I was able to find this form
online that you can print, fill out, and send it. But please read up on the
entire program before following through to make sure you are eligible and follow the procedures properly.

That is terrible! Was your console laying flat or set verticle? Was it bumped at all while running? Just trying to figure out what would have caused it.

it could have been dropped and cracked a little on the disc, then when you put it in ,, the spinning of disc might have caused it to break through and fly off. You might be able to get a new disc from Activision , might be under warranty or a defect,
