For the first blank space you can enter the email of your account which was stolen or perhaps the phone number with which it was affiliated, for the second blank option you must enter a new email to which you have access since there you will receive any positive or negative information regarding your account.

For the first blank do I put the hackers email? Or the gmail that I had that got hacked?

For the second blank, Do I put a brand new gmail I created?
