Why my xbox 360 won't read halo 4?

Hi, i've got the 250GB version of the xbox 360 E and it can't play the physical disc of halo 4. I've taken a close look at the back side of it and there are no scratches and it looks like new.

The disc could be played successfully on my xbox, but one day out of the blue my xbox didn't play it anymore. When I try to reproduce it there are no error messages, the screen just goes black for a few seconds, then it says "disconnected from xbox live"
and finally it takes me back to the xbox dashboard.

So what do you think?

Should I buy a new disc or is it better to purchase the digital game from the Microsoft store?

System error!!

I tried to set my my boyfriends new xbox one. I followed everything. It said something about an update. I did it. Now it goes to a black screen and says system error. I can't press any of the buttons on the controller?

I have a region locked with black ops 1, I played it normally in my 360, then an update came and the game started to get problems, then the disc couldnt be read, even i cant download the black ops demo couze it says something about the region, i thought
it was the disc, but i try to use it in two differents xbox and the same problem...

I am also experiencing the same problem with the recent update. I can't play COD Black Ops 2 because it will just freeze up and the screen goes black. When I try signing in it say some content is not available at this time error 807b0191. My Xbox is pretty
worthless at the moment because I can't do anything on it. Any suggestions?

me too i redeemed my 48 hours on the website and on xbox it says 2 days trial but when i play on black ops 2 it says The current profile is not allowed to play on Xbox live but on black ops 1 the online works

Yeah i have the same problem. Everything worked until i applyed that new update. just black ops/mw2 basically wont play in my xbox anymore. And they played Fine the day before.
