    Something curious happened to me. I tried to install the hub app through the Xbox One Store. I only got it after I bought the app for $ 0.00. I found this way of getting the app strange. I hope I have helped someone who is unable to install the app.

    Yep, conducted a repair install ...issue remains!

    Tried installing from the MS Store (install using Xbox) and tried installing direct from the xbox app. ...issue remains.

    Yep, reported issue on Windows feedback Hub.

    Hi Erelyn,

    You can
    report issues on the Xbox One through the Xbox Insider Hub app    . You just need to install the app from the Store and then hold the Xbox button to pull up the power menu and you should now see "report a problem". The Insider Hub app will open and take you
    to the report section where you can put the description of the issue along with screenshots like the ones you provided here.

    Press Y on the homescreen of your XBOX ONE and search in the search bar for XBOX-Insider-HUB and install it.

    Now you can press and hold the xbox button on the xbox one Controller (like if you would to want to turn off the controller) and select report Problem.

    If you've not done so already. I would suggest and recommend that you report the problem threw the console directly after you experience the issue. By submitting a report on the issue. You can help pinpoint the cause of the issue.
