1 Shepard had teleported up to the Citadel the environment was a mish mash of backgrounds of the Shadow Brokers Lair? Alot of games use backgrounds from their previous games. Dead Rising 2 was made using actually everything from Dead Rising 1.

2. Also why was the Catalyst in the image of the boy Shepard had seen on Earth at beginning of game?

We do not know exactly what this AI is capable of. We know he has a past history of watching and evaluating synthetic relations with organics. Perhaps by blending in he is able to do this.
