Even when the game is offline, correct? If that is the case, this could already be a hardware problem with the console since you have tried changing monitors and cords as well.

What display are you using and how big is it?

The Xbox Series S is largely a 1080P console. There are some games that can run at higher resolutions but not many. If you have your console connected to a large 4K display, the image will be stretched by the console to fill the entire screen. This means, if you're playing lower resolution games, Xbox One titles or a backwards compatible 360 game, they're not going to look too great. They will have soft and pixelated look to them.

1. Press the Xbox button to open the guide.

2. Select Settings.

3. Select System.

4. Select Console info

5. Select Reset console.

6. Select Reset and keep my games & apps

This will also help sync the data on your console properly.
