Also, try to verify if the subscription is active on account.microsoft.com > Services and Subscription.

if that is the case then your account will have a Past Due Balance which can't be paid using pre-paid cards or codes.

Yes it was one of those Vanilla Gift gift cards, and I still have the card.

Xbox did refund me the $1.01 I used to pay for the game, but the rest of the game was paid for by the card, which I didn't get back.

I paid Xbox Live Gold without credit card and only with balance that I have. In addition, Xbox Live Gold allows the desactivation of periodic billing.

I'm afraid that subscriptions can't be paid via Account balance (Microsoft Gift cards) you will either need to use a payment option (like a credit card) or a prepaid card for Xbox Live or Xbox Game pass.
