I have a reoccurring bill on my bank account from Microsoft every month on a lost account,...

No, I removed it two months ago and then got a billing for £10.99 from microsoft. (UK price of xbox ultimate subscription)
When I logged into the xbox and on the microsoft xbox website my account information only showed a old expired bank account that I got a 0x error when i tired to remove previously, my account information did not have my current and working bank account which i had removed.
Yet I was billed which i found out about because I get notifications from my bank app. Yet my account didn't have the subscription active and i didn't in fact even have a xbox at the time, I bought one mostly to have a play around on this weekend which is how i found out the ultimate wasn't active and reoccurring billing was set off anyway.
So Microsoft had my account information, billed it not even on the proper date for reoccurring billing and that information wasn't visible from my account since I had removed it and it wasn't showing before i put it back in to repurchase the xbox subscription. I talked to someone on the phone and he processed the refund and was able to sought it out. so it's not a money thing I have xbox ultimate and I'll be getting the refund in a day or so.
My point here is why was I billed in the first place, at this time. And why did my xbox and microsoft account not show my active bank account on it regardless or reoccurring billing?

If you want to remove your bank account info on xbox you have to turn off recurring billing on game pass and it will stop billing your account every month than you can remove your bank account from the xbox. Hope this helps.

View your Xbox purchase history online

If you see an Xbox charge on your bank or credit card statement that you don’t recognize, you can review your Xbox purchase history online.

1. Sign in to the Payment & billing page with your Microsoft account.
   
   Note If you don't know your Microsoft account email address or password, use the Lost Account Solution or the Lost
   Password Solution.
2. Compare your purchase history to your bank or credit card statement.
