You need to recover your gamertag to your console to update it. It doesn't sound like it is seeing the new family pack. And for the accounts you want to add, you need to recover them to your console, then add them. Once they're added, they can be recovered
again to a different console.

Yes, each account is one per user and yes, each member would need their own subscription to access Gold features (on Xbox 360, this works differently on the One).

You can add a child account by creating a new account from either the console or the web, just as you would have done for your own. When you designate the age as under 18, you will be asked to associate the account with a parental account. At this point
you should use your own details (the same as for your profile) in order to tie the child account to your own.

I should note too that for the first three accounts created from any Xbox 360 console, the new account gets the option of getting a free 1 month trial of Xbox Live Gold. This is not available if you 1) created more than three accounts from that console or
2) create the account on a PC, or any device other than an Xbox 360.
