Kindly make sure that your setting is set up correctly. Please follow the steps below:

"Press the Xbox button on your controller to open the guide.

Select Profile & system > Settings > General > TV & display options.

Select Video fidelity & overscan.

Under Display, select HDMI."

If that didn't work, please try using a different HDMI Cable or try using a different TV just to test it out. You can also check this link:

When you're in the Xbox Music app itself, and you go to the top right corner to open the 'Charms bar', then click Settings, is the volume showing 'not-muted' or at zero there?

Mi question here is , why even without a controller (power on the xbox only) , the muted icon is showed on the xbox? i already verify the headphones on my lap and works fine with skype.

Im thinkig this is some problen with the xbox (software or hw) , thats why i insist on continue the troublehooting with this forum

Even when your policy is to mute them Immediately. You can't always do it, I've had to rip my headphones of my head during loading screams countless times, just to keep my ears from bleeding.

Then there are those infuriating moments when they start making strange loud noises or singing some katy perry song at the top of there lungs mid game. And I get killed in the process of pressing select and muting.
