Headset Leaking Audio Even While Muted

Once the headphones are connected, double-tap the guide button on the controller, and navigate to the bottom for headset settings.

You should find an adjustable headset volume setting. That will be the game volume in your headphones.

Please note that audio will still play from the TV even while headphones are connected to the controller. You can just mute your TV volume while using the headphones.

If using headset for game audio and chat, mute the TV or any device your game and chat audio is coming through and make sure 'chat' is set to output through headset only. If you have your tv volume up in the background while you're on the headset there
will probably be echoing especially if you have your party audio set to output through your TV.

When you plug a headset into the controller, it doesn't automatically mute the TV speakers. Both audio sources (headset, TV speakers) continue to play. You may have had the TV muted or HDMI/Optical audio disabled while you were using the headset and troubleshooting
your audio.

Check to see if the microphone on the headset is muted. Some headsets have a mute button on the headset itself or on the cable.
