Having problem with my new Xbox One S

So i finally bought a Xbox One S, and it seems that all games I tested suffer from the same problem..

For example, when I play The Witcher 3, the trees, the shades, torchs, and another small objects that are far away from Geralt, take a feew seconds to load. This problem repeats in almost EVERY game with objects far away from the main character (especially
open world games). Another example is when i play Dead Island, and i see the floor and even some ZOMBIES rendering in-game.

It's a a little bit annoying play a game with these graphics problems, and i really dont know what to do. Any help?

It's all consoles, not just yours. My OG One and X both to that. Some games like The Division 2 and Far Cry 5 are really bad.

Other games if I remember correctly don't have that problem like Forza games.

Which xbox´s do you guys have,the newer S-version,or the old one?

I had disc-reading problems until the dvd-rom crashed on my old xbox,nothing on my new one,worked fine for almost a year now...
