I've been having this problem myself, and it definately sounds like the controller is really the issue.

I have one of the original xbox headsets and the headset is fine-- but the fancy, glowing controller my husband purchased (used) has faulty wiring, glows when it wants to, and vibrates at weird times when the setting is always off! Sometimes, even the littlest
bit of liquid or a good thump (by accident or with gamer gusto) can do considerable damage.

Try taking the controller and original headset with you to your nearest gaming store if you're able, and have them try another controller with the headset for comparison. Hopefully the new headset is more comfortable--else you can always use it as trade
for a new controller.

When you connect the headset into the controller, press the glowing Xbox button on the controller, navigate to the "System" tab on the right, and select "Audio".

In the Audio settings, you can adjust the headset volume.
