Fallout 4 - Achievements not unlocking after upgrading from a Xbox One to Xbox Series X

So far, from the details you provided, you can try the following steps and let us know the result. You can just skip those you have done already.

Remove and readd profile then proceed with the troubleshooting.

If a completed achievement hasn't appeared online after 72 hours, your console might not be connected to the Xbox network. If you completed an achievement or challenge while offline, you won't receive related notices or rewards until you connect to the Xbox network.

1. Make sure that your console is connected to your network equipment (such as router, modem, or gateway), and then press the Xbox button  to open the guide.
   
2. Select Profile & system > Settings > General > Network settings.
   
3. Verify that your console is connected to the network. If necessary, select Test network connection to get a diagnostic.
   

After connecting to the network, try unlocking the achievement again. If you experience issues trying to connect to the Xbox service, see:

Solution 2: Check the Xbox service status

Check the Xbox status page. If you see any services with alerts, expand the service, scroll down to Notifications, and sign in to receive a message when the service is up and running again.

Solution 3: Check your achievement progress on your console

When you unlock an achievement, you may not see a notification or an updated gamerscore right away. The Xbox service must validate your achievement first, which might cause a short delay.

If your gamerscore isn’t updated online, verify that it's updated locally on your Xbox console.

After your gamerscore has updated on your console, verify that the changes have synchronized to the Xbox network. To see your achievements on your console:

1. Press the Xbox button  to open the guide.
   
2. Select Game activity > All achievements.
   
3. Select the game in which you unlocked the achievement, and then view the unlocked achievements list.
   

If you completed the requirements to unlock the achievement on your console, but it doesn't show, it might not be synchronized with the Xbox network. Allow up to 72 hours for this synchronization to take place.

Solution 4: Check your achievement progress online

You can check the progress of your achievement online to verify that your console has synchronized with the network.

To view your achievements online:

1. Sign in at Xbox.com.
   
2. Select your profile icon in the upper-right corner, and then select Xbox profile > Achievements.
   
3. Select the title of the achievement that you're trying to unlock.
   
4. If the achievement appears under Achievements, it's already unlocked.
   

If the achievement appears under “Locked Achievements,” either the achievement was not successfully unlocked, or the console hasn’t synchronized with the Xbox network. Allow up to 72 hours for this synchronization to take place. If it hasn't appeared after 72 hours, try unlocking the achievement again.

Solution 5: Restart your console

Restarting your console ensures that any achievements awaiting synchronization on the console are re-sent to the Xbox network:

1. Press and hold the Xbox button  in the center of your Xbox Wireless Controller to open the Power Center.
   
2. Select Restart console.
   
3. Select Restart.
   

Once you've restarted your console, allow up to 72 hours for the synchronization to take place

Thank you for your update. I believe it is pass 75hrs, are you still having issues?

Yes I am. This did not solve the issue. Whats the next recommendation since everything above hasn't worked?

I have done everything on this list outside of delete and re-downloading my gamertag profile. I'll do that and report back if it works. Thanks for your suggestion.

EDIT - I just deleted my profile and added my profile. Loaded up Fallout 4 and still no achievement pops. I'll give it 72 hours and report back.
