Kindly try clearing your console's cache by following the steps below.

1. Turn off your console by pressing the Xbox button on the front of the console for about 10 seconds, until it shuts down completely.

2. Unplug the console's power cable. Wait 10 seconds.

Important Be sure to wait at least 10 seconds. This step resets the power supply.

3. Plug the console power cable back in.

4. Press the Xbox button on your console to turn it on.

If it still doesn't work, try a wired connection and make sure your Xbox is updated.

If it's the same issue, try to refresh your operating system and delete all potentially corrupted data that restricts you from using your console normally without deleting your games or apps.​​​​​​​

NOTE: Make sure that you know your account logins before doing this step. Also, this will remove all your Home Xbox associations.

1. Press the Xbox button to open the guide.

2. Select Profile & system > Settings > System > Console info > Reset console.

3. On the Reset your console? screen, you'll select Reset and keep my games & apps.

Every time I start up forward horizon 4, it just won't let me press A to start the game and then It says controller disconnected on my Xbox series s, what can I do to fix it?

I would avoid Xbox Series X/S controllers. My first controller that came with my Series X developed that same issue. After ignoring it for a while (because I play shooters where "A" jumps once) it developed a stick drift. It was internal, and had nothing to do with the actually stick, I later found out. Then every button started double pressing, triple pressing. After figuring out I couldn't get it replaced, I disassembled it and made sure everything was clean and clicked into place. It was. Just blew through ANOTHER Series XS controller after less than 4 months. This time my left stick can't reach full velocity when pushed forward, (which has nearly killed me in Resident Evil, like a thousand times).

So, based on my experience, I can tell you this is very normal for this particular xbox controller.
