This has come up before I believe in a similar post,

There might be some hardware issues impacting your controller and may need to raise a tech ticket to have the hardware fault investigated or take back to place of purchase if not online and see if a warranty swap and see if a new one works

Just did that. Thought it would work but it didn't.

When connected with USB and the wireless adapter is on and connected, when you unplug the controller does it turn off or does it connect to the wireless adapter?

If it turns off after unplugging it from USB you will need a new controller.

are you trying to connecting it wireless or wired?

If you are trying wireless, make sure you have the V2 controller and bluetooth support on your PC.

You don't need a V2 controller to connect it wirelessly a standard controller works as well aslong as you have the wirless adapter for windows 10

So after months of preparing myself to contact Microsoft, I finally stumbled upon the answer to the problem I had. I tried multiple officially licensed controllers including Elite, Elite Series 2, both xbox wireless adapters for windows... I was
at my wits end.

Go to All Settings, search for "Turn Airplane Mode On". If you'll kindly look at the bottom where "Xbox Wireless" is in the off position, turn it on. Now your controllers will pair correctly with the Xbox Wireless Adapter for Windows.
