    BO4 is the only game I play so I always leave the disc in the xbox. Every once in a while I would get the following error:

    Insert the game disc

    We need that to read the license. If the disc is already in, check to see if it's dirty or scratched. (0x82d40004)

    I would take it out, clean it and put it back in and then the game would start.

    Recently though that does not work. The only way I can get the game to start is by physically unplugging the xbox, wait for the cache to clear, plug it in and then restart the xbox. Once this is done the game will load. I don't have this issue with other
    game discs because I tried them recently.

    I did a "soft" reset of the xbox, which was told to me to do by one of the expert gamers, this unfortunately did not work either.

    Does anyone have any other ideas what can be done to fix this? It's a pain in the neck to have to unplug it every time.

    I would suggest you remove the disc after you have finished playing the game and not leave it in once you switch off the console.

    As this can cause many problems, as you are experiencing now.

    I just unplugged, waited a few seconds, and plugged my XBOX ONE S back in and I was able to sign in again.
