A. Reinstall your controller driver by right-clicking the Windows button > Device Manager. You may want to check the “Controllers” or “Universal Serial Bus Controllers” to see if the affected device appears. (Expanding the categories will help you).
You can uninstall the driver by right-clicking on it once you find it > uninstall device > uninstall. Then, restart your PC, connect the controller and try again.

B. In case you have Steam installed, please disable Steam controller input via Steam settings before trying again. The same applies if you have any other controller mapping tool.

C. Perform a clean boot in your PC, once restarted, try again.

Hi, first try resetting your console by holding the power button on the console for a full 10 seconds until it turns off. Unplug it for a minute. Plug it back in and try again. See if it works properly.

I have the same problem with two different xbox ones and media remotes.

Actually, that's a standard wired Xbox 360 controller. They have a breakaway design. It sounds like you're missing the other end of it.
