Black Ops 3 disk not working says I need to insert disk even thought disk is in!!!

So frustrated, bought game on release day and has worked fine until I went to put it in tonight. Says put disk in but disk is in and says to insert and asks if we have right to play it. Yes we do I paid for it. Game was bought at game stop. What is going
on???

Black ops 3 wont initialize

We have had our 1 and Black ops 3 since Christmas. We have never had any problems until recently. My son went to play and he got an error saying that we needed to insert disk. Well the disk had been in the entire time. For some reason when we put black ops 3 in it wont even recognize the disk. Madden16 and Deminsions both work just fine but our BO3 disk acts like its not even being read. Live is working good our internet is fine the disk itself has no scratches or smuges. I'm at a loss as to what we need to do. Please help, I deleted the game from the console to try and reinstall it but the game disk doesn't read at all.
