You would have to try submitting a case review at http://enforcement.xbox.com If the enforcement team overturns their decision you will be given the chance to change it. Otherwise,
you will have to pay if you want to change it again.

Without a doubt that gamertag would be flagged as offensive, You won't be able to get it back and to change your current assigned gamertag you would need to pay $10, Theres no way around the gamertag change fee after a forced gamertag change.

There is no way to request a forced name change. The enforcement team will only investigate your account if someone has filed a complaint on your account and they will only give a forced change if your gamertag violates the
code of conduct. Even if they did issue a forced change, they would give you a randomly generated tag. You would not be able to choose your new tag without paying.

If you were issued a forced name change, it was because your gamertag violated the code of conduct. The COC clearly states that you will be given a random name if this happens, and serves as your only warning. The policy of allowing you to choose your gamertag
after a FNC changed and you will now have to pay a fee to change your gamertag to something of your own choosing.

in addition to what xTye already mentioned, be aware that a forced gamertag change still requires you to pay for a change to a gamertag you like after it was changed to the generated one.
