Xbox Series X Controller disconnects when playing certain games

So I’ve been having this issue since Vanguard came out. Whenever I would play Vanguard or now just recently Dying Lignt 2, whenever I have a headset plugged into a controller (I’ve tried a combination of 3 different headsets and 2 different controllers) my controller won’t let me do any inputs. Like I can still hear what people are saying to me but I cannot move or look around. The light on my controller still stays on as well. So whenever I have this issue I’m forced to take out a battery and put it back in. Any ideas are much appreciated! It is very annoying to deal with this!

Connecting the controller with a USB cable doesn't work,

Controller still disconnects even with a USB cable connected,

I have the same issue with Series X using mouse and keyboard for Call of Duty Vanguard.

I played during the beta and there was no problems with the controller disconnecting but as soon as the game released, the controller disconnects,

I've tried a Xbox One controller Series X controller, with batteries and a USB cable, and the controllers still disconnects.

Can you open the accessories app on your console and please share the firmware number of the controller you are using? Also are you in the Xbox Insiders?

Thank you for the promp response. No, it is not via bluetooth. I only play with the controller on a cable into the xbox. I do not play wireless. What is odd, as I explained earlier, my options button is not working at all, and it seems over sensitive when not in a game. It is very annoying as I cant select certain things due to this, and in game I can not access certain functions, as the option button is not working at all. Im on an Xbox Series X with an Elite V2 Controller

I'm playing on a PC, this isn't a Forza issue, it's a Windows/Firmware/Compatibility issue, the controller disconnects even if i'm not in any games.
A lot of people have this issues on PC, for some it works fine in some games, and in others it keeps getting disconnected, and for most it just disconnects regardless of game or not.

It's been an issue since Xbox One controllers, and day Series X

Some times both consoles are turned on at the same time but it happens when only one is on as well

Yes both share same network

All 4 controllers are normal xbox one controllers

They disconnect when playing online and offline games

No pc games

First started happening maybe In September

Also I have a Xbox series X In a Third room that has no issues
