Xbox Series X controller disconnects randomly mid-game

You mention that the console is updated, but also make sure that the controller is updated, you can do this through the "Xbox Accessories" app.

Additionally, try resynchronizing the controller and the console with these steps:

1. Press and hold the Power button on the front of your Xbox until the console turns off.

2. Unplug the power cord from the back of your Xbox.

3. Wait a minute, and then plug the power cord back in.

4. Press the Power button on your Xbox to turn it back on.

6. Press the sync buttons on both devices at the same time.

Connecting the controller with a USB cable doesn't work,

Controller still disconnects even with a USB cable connected,

I have the same issue with Series X using mouse and keyboard for Call of Duty Vanguard.

I played during the beta and there was no problems with the controller disconnecting but as soon as the game released, the controller disconnects,

I've tried a Xbox One controller Series X controller, with batteries and a USB cable, and the controllers still disconnects.

Select Settings > System > Console info > reset then keep my games and apps.

It's worth a shot though but to best isolate the issue, have you tried any other controller? I asked because if it happens to other controller, I'm afraid the console needs a service.
