Xbox one Stereo Headset adapter quit working ob me

Are you still running into issues with the headset?

Have you been able to test that headset on another controller and on another console at this time?

If so, what were the results?

Have you by chance used that controller with a Windows 10 PC before the issue started?

I had a similar issues, but with the wired headset. After re-updating the controller [an original controller at that]... endlessly... I could not get it to work.

The only way I found to resolve it, was to purchase one of the brand new Xbox One bluetooth wireless controllers, and plug the headset directly into the controller's 3.5mm jack (tossed the adapter). And, to use the Windows Sonic audio output. It's a horrible
solution considering my original controllers were working perfectly up until then.

We have two of the
Xbox Stereo Headset with Xbox One Stereo Headset Adapter, and both sets have worked well with no problems for sometime now. And I have used the
adapter with non-Microsoft headset with no issues. So, given that you say your "3.5 mm cabled headset " works directly connected to the controller, then I regret to say it would seem that the fault lies with the adapter itself and may need to be replace.
