Xbox One Stereo Headset Adapter controller update fails.

I cannot get my controllers to update as soon as I plug in my adapter to my Xbox One controller. I have followed every instruction I have found in Xbox.com and from the instruction paper that came with my device. Anyone else having this issue? I have updated
my controllers without the adapter without a problem. I am able to hear sound through my headset connected to the adapter and use the mute and volume buttons but still have issues. I get the update prompt all the time I connect my headset to the port and it
fails and takes me to the Error screen with the controller and green exclamation mark next to it stating the update failed. I also cannot turn my Xbox one with the adapter in place on my controller. The white light just blinks on controller but the Xbox does
not respond by turning on. Any solutions on getting my controller to update.

How do we update the controller? NO USB Cable comes with Xbox One.

About updating controller, To use the Xbox One Stereo Headset Adapter with your Xbox One Console, you’ll need to update your Xbox One Controller with the latest software to support the Adapter.

So Titanfall controller update is done by over air, no cable, (February update) and Stereo Headset Adapter controller update you need an cable, which comes with the adapter (March update).
