Xbox one stereo adapter chat issue

I got a feeling nothing can be done. but I'm hoping I'm wrong.

My issue is the chat stops working randomly and no one can hear me.When it happens the controller will disconnect for a second then reconnect on it's own.

• What accessories are you having trouble with? Stereo adapter. Xbox one controllers (forza edition and convert ops camo edition)
  
• Describe what is happening. No chat audio from me. I can hear others usually.
  
• What have you tried so far? Please don't say "everything!"
  in a xbox party and chat is working then all the sudden xbox controller disconnects and reconnects, then no ne can hear me.
  
  The Xbox party chat shows no microphone detected.
  
  occurs randomly, but usually once issue starts it will keep happening every 2-3 minutes at the longest or as often as every 15-30 seconds.
  
  If I shut down the controller and start it back up the voice will connect and then may disconnect again within seconds to minutes.
  
  Plugging direct to the controller does not resolve issue
  
  removing batteries, plugging direct to console then powering controller on sometimes makes it work

Party Chat cannot hear audio

First, the reason I posted this under Xbox One console is because I swapped my accessories with my son's and they work on his xbox one and his accessories didn't work on mine. so the issue is not chat adapter, controller or headset

Xbox one original controller, stereo headset adapter and headset and I can hear in game chat but as soon as I connect to party chat, they can hear me but I cannot hear them. I tried the original headset that comes with the xbox one and it worked so I figured
it was the stereo adapter. so I purchased a new stereo adapter and have the same issue. so I purchased a new controller with 3.5mm jack. I have the same issue. but if I use my headset connected to the 3.5mm jack on the new controller party chat works fine,
but doesn't work connected to stereo adapter (and I tried 2) So I thought I would try on my sons xbox one (we have 2 xbox ones in the house) so we swapped headsets , controllers and stereo adapters and I had the same problem on my xbox one. whereas his was
working fine (and he was using my controller, headset and adapter) So it would seem to be an issue with my console, that is the only hardware difference. any ideas?
