Are you running on an original Xbox One, Xbox One S or Xbox One X?

On the original One or One S, the game might only reach 60 FPS in some areas. So no matter what monitor you have, the game will probably run at "up to 60 fps" depending on the environment and how much action is happening on screen.

Also be sure to turn off the frame limiter in the game's settings.

I am having the same issue. I can turn on the xbox via the console or controller. All looks well but once it reaches the menu screen the tv blacks out. The tv isn't the problem as I have checked the same hdmi port with another device. The only thing I
can do is shut the console off through the home button.

The same thing has happened to me. I have tried my controller and the Xbox One Smart Glass app, but the home button does nothing. I have another Xbox One console and it has happened to that one as well. Nothing related to the home screen functions once a
game is started, so I have to turn my Xbox off and on every time I want to switch games or apps.
