Xbox One going black screen when trying to play a game

I have tried everything I can think of and some suggestions seen on forums for other game disc reading errors and nothing has worked, every time I try to play Titanfall which is the only disc I have for the One so far it shows the beginning picture then
simply stays stuck on a black screen, no error message or anything.

Stuck on loading screen

Yes!! What is going on.? Should be simple, insert disk, install , play game. However, i get thru install and end up with a black screen when trying to play the game. What a terrible customer experience. WTF Microsoft!?!?!?

Xbox one S not playing bluray disks and games from disk crashing sometimes

Got Xbox one S 2 days ago won't play DVDs or bluray disks. It film in and it won't play goes black screen then displays picture for a second then goes to black screen keeps trying to change screen resolution to no avail. tombraider from disk sometimes works
sometimes freezes when loading also

Clear the persistant storage on your console, disconnect your network, uninstall titan fall, turn off your console, unplug all the cords connected to the console, hold down the power button for 30 seconds, plug everything back in, turn back on the console
(don't forget this step), start to download the game (yes, disconnected from your network). After it has finished downloading, connect your network, and perform the game update.

Bam, it should work.

UPDATE: I am still not able to actually get to a game in the Xbox One MLB app. I let it sit at the black screen for probably five minutes until it finally kicked itself back to the scoreboard screen. No video was playing in the scoreboard view. When I tried
to select the game again it asked me to choose a feed and then it just stayed at the scoreboard screen and didn't even try to go to a black screen or anything.

What do you see when you start to play Titanfall? Black screen? Error message?

If you see a black screen try this -
support.xbox.com/.../power-cycle-console

Then try Titanfall again

Tried a different HDMI console and still get the screen going black every few seconds when playing a game and when on xbox dashboard. Anyone any ideas.
