Xbox One Freezes - No fix - Urgent help needed please

I'll try and explain this as well as I can even though it began to happen months ago and I really should have gotten some help by now. I got my console in summer 2015 but Im not sure of its model

So this all started when after one time of turning my xbox on (maybe around one year ago now) it got stuck on the green start up screen and never fully booted up. I left it for hours and nothing. I finally just yanked out the power cable and retried it - this
worked for now...

Recently, in maybe the past 6 months, my xbox has started freezing during games and when on the dashboard. It also sometimes freezes on start up on the green screen.The freezes, when I am successfully booted on the xbox and past the green start up screen, typically
last between 5-60 seconds and when it happens, it occurs maybe every 10-20 seconds (so very often and impossible to play a game on.) The only fix for this is too hold down the power button and reboot the xbox. There has been a number of occasions where after
waiting maybe 30 mins nothing has happened and the xbox has remained frozen. I tried turning the xbox off but the xbox power button would just flash at me and even though my screen would go black as if turning off, the xbox wouldnt fully turn off. The only
solution would again be to disconnect the power brick from the back of the xbox and force a shutdown due to lack of power. In recent times the full blown freeze has happened, I have held down the xbox power button (I think this is called a hard reset or shutdown)
and this successfully turns the xbox off but doesn't fix the problem. Sometimes during this freeze I can navigate home and quit the game, but when I relaunch the game it normally gives me an error message and I can not play any games. By this point im normally
very frustrated and just turn off my console. Its important to note I am always connected to xbox live during these freezes and I can always continue to talk to the people I am in parties with. I mainly play Fifa or Battlefield and here are
other glitches I have noticed;

Fifa menus not loading correctly (e.g player faces not loading on FUT cards, when navigating menus the headings appear but the actual content does not)

Fifa commentary not keeping up with gameplay and sound not in sync (e.g I took a shot five mins ago and the commentator has said nothing since and will then say "good shot" followed by the crowd reaction - so basically the game sound is delayed and not keeping
up with gameplay)

Spawning into battlefield one but my loadout not appearing - so I am walking around with no weapons and can not play the game

Textures not loading on certain games and falling through sky or roads disappear e.g GTA V

What I have tried:

Unplugging all cables, removing discs, basically shut down xbox for periods of days then trying again

Removing xbox live account and starting again

Factory reset - I have tried both versions of the reset where games are deleted and where they arent. The most recent I did was two days ago & I deleted all games - this has made the problem worse as half of my games are yet to fully install and the xbox
is taking an obscene amount of time to both install them and update them.

The fact that there is so many different issues is really concerning for me and I would appreciate ANY advice. Its most certainly not going to be fixed by an update as this issue has been going on for many different console updates. I live in the U.k so
if I do need to send it off for repair any information I can be given will be really helpful.

That's usually the sign of a region issue. If your account region does not match your physical location, or your codes are for a different region than your account, you will get that issue.
