Xbox One controllers keeps taking me out of the game

When I play Assassin's Creed Unity, every time I press the YBAX buttons and the joystick at the same time, it suddenly takes me out of the game to a random option at the xbox menu. The most recurrent one is the TV/Cable option. Does anybody knows why this
happens? Is it a shortcut that I'm not aware of? I'm definitely not pressing the xbox home button.

Controller's Right Trigger is Pressing Down on Its Own

Okay. So I have went through 3 Xbox One controllers in my time on the One, and 2 of them have been for the same reason. The reason is rather unlikely, and I have not seen much support posts about it. But the right trigger in both of the controllers
is completely broken. The One keeps registering the trigger as being pressed, but I am not pressing the trigger down. What also keeps reoccuring in both of these controllers is that the top left screw in the back of the controller handles, always comes out
for some reason. I was wondering if anyone has been having these issues and if there is some way to troubleshoot it. EDIT There is also one detail I forgot to add. Both of the right triggers can easily be lifted out of place.

Is it only happening in one game or all of your games? Is it digital games or disc games?

If you game crashes at the dashboard of the game it could be a couple of things but I would try a

Reset to factory defaults.

There are 2 options a full reset and a soft reset. If you don't want to redownload your games. Select keep my games and apps.

This should help you roll back any bad updates and do minor maintenance on your console to get rid of any corrupt file or anything that shouldn't be there. Make sure you know your emails and passwords to your account before troubleshooting to make sure you
don't lock yourself out! Reply if I can help out.

This happens to me a lot. What I do is take out my battery and out it back in. It might take several tries to keep your controller on. Hope this works!
