Xbox one controller randomly disconnects, stops headset from working

I would be on the xbox and chatting with friends and my controller will disconnect and immediately reconnect at random points. This causes my headset that's plugged in to stop working. It takes me at least five minutes of turning the controller on and off
again to get the headset to work and it constantly happens. Sometimes it happens even if no headset is plugged in.

I've updated my controllers and changed batteries and power-cycled the xbox and it still doesn't work. My two other controllers have the same problem, so I can't use them either.

What the heck is happening? And how do I fix it?

Reporting 2 bugs in the Xbox One. Cntroler/ Stereo Headset

1st) When receiving notifications the Xbox button no longer functions correctly. When answering the notification by holding the Xbox button on the controller, the controller now turns off instead of answering the notification message.

2nd) Xbox One stereo headset randomly stops working. Have to sign in and out of Xbox Live to get the stereo headset to work again.

Turtle Beach static / not working

No one else has issues with a headset stopping the controller from working? It will actually disconnect and say, to reconnect controller while playing. Batteries are charged fully.

This issue was resolved by replacing the headset. Once done all the quirks I was experiencing went away.

If your controller works fine with the headset unplugged, but does some odd things like randomly disconnect, not "see" the xbox when turned on, randomly switch to kinnect for chat... I strongly suggest replacing the headset.

Both mics are xbox official headsets and controllers, one of the controllers is a day one edition but just bought another brand new official xbox controller to see if that was the problem. The Mic just randomly stopped working one evening. And yes the
mic symbol is shown in the top right hand corner with no line through it

Turn off and disconnect all controllers from the Xbox 360 and disconnect the wireless headsets from the controllers.

Connect the wireless headset to the Xbox 360 using the USB cable. Turn on the wireless headset.

Sync the headset to quadrant 1 by pressing the "Connect" button on the on the Xbox 360. Then press the "Connect" button on the wireless headset for two seconds. Do not continue to press the "Connect" button on the wireless headset, as this will change the
quadrant to which it is connected. If the headset now works without the controllers attached, the problem must be with one of your controllers. If the headset still does not work, then the problem is with the headset.
