Xbox one controller keeps disconnecting and reconnectiong every 5 mins

I'm sorry to hear that. Here are the steps that you can try for your concern.

Solution 1. Do you have an external hard drive plugged in to the console right now? Or wireless headset and any other accessories other than the controller? If yes, kindly unplug those and check if the issue still persist.

Solution 2. Try to do a soft factory reset without removing any games and apps.

Press the Xbox button to open the guide.

Select Settings > System > Console info > reset then keep my games and apps.

NOTE: This will delete the profiles on your account, make sure that you remember the email and password to your account.

Because the NXE Update? I have also the Halo 5 Xbox One and the issue with random disconnects starts after the NXE Update on Nov 12. My old controller is working, my new halo controller isn't (disconnected every 30-45 min)
