    I've been having this same issue since I've received my chat adapter. I even purchased a new controller and did the update thinking it might be an issue with my day one controller. Guess what, I was wrong. Same thing keeps happening and it's really pissing
    me off. MS come up with a fix for this!!!!!

    LISnowman / BCardina , did you tried my suggestion a few post above? It worked for me... no more freezing controller.

    WizR: I checked my console, and it looks like I was still at stereo uncompressed already, so I'm not sure that's going to be a permanent fix either. Xbox One controller disconnects when headset/and adapter plug in :( Hopefully it works for you, though!

    I have pretty much the same problem with no idea of a fix. Got new adapter, plugged the adapter into my controller and Astro A50's, message pops up to update controller, plug in my USB cable and the controller just shuts off and I can not get the update
    because controller must be on but it refuses to power on when both usb and adapter are plugged in

    Same thing is happening to me. I get a horrible buzzing noise in the headphones, and a notification that the controller is disconnected, but the controller is still lit up. I have to hold the menu button for a few seconds to reboot the controller.

    This has happened while playing forza, and while watching a bluray.

    I'm using the headset adapter and razer kraken pro. No charge/play kit, just fresh AA batteries.

    I tried and it was already set for stereo. Tried all 3 settings and non of them helped solve the problem with the adapter. Thanks for the idea though.

    I've been having similar problems myself - also been in constant contact with @XboxSupport on twitter.

    I updated my controller in order for the headset adapter to work with the controller - since that when playing Titanfall I too get random disconnects. Luckily mine only appears to happen within the first 30 minutes of playing but it's still annoying/frustrating
    to say the least.

    I have tried several things suggested by @XboxSupport the first and most common response is to try a power cycle - done this several times to no avail.

    The second response was to try running the update again - which I have done several times, before and after a powercyle again to no avail.

    I have also tried with the controller directly connected through the plug & play cable that comes with the play & charge kit. I don't appear to have any drop outs with this method as all data sent from and to the controller is now going through the USB cable
    however I have not played as many games while using the USB cable.

    Another thing I also tried was using the standard Official Xbox Headset that came with my Xbox One console - again I appeared to have no drops outs while using this headset which is limited in comparison the the headset adapter.

    Finally they've asked me again to do another powercycle following specific steps - which were the same steps I carried out in the original update to the controller and have done several times since as mentioned above.

    I hope there will be a fix for this soon as from what I've tried above it only appears to happen WHEN the Headset Adapter is being used on a WIRELESS controller. I do not plan on gaming permanently connected via USB to overcome the problem temporarily as
    the whole point of having a wireless controller is to be just that - WIRELESS.

  11. My Controller tends to disconnect during the heat of a competitive battle. I plug in my play and charge kit, people complain about the feedback it gives off... *sigh...

    My controller worked fine until the battery suddenly died. I chose to switch controllers. When I plugged into the head set adapter I could hear my party but I could not communicate. Later, when I switched back to the first controller the headset worked
    but Xbox Live put my in some kind of other group within my part. So they still could not hear me.

    Both controllers received the same firmware update using the same procedure at the same time. So there is no excuse for only one to work. The Xbox Party segregation is just an example of how much the Xbox Party system needs more work.

  15. There is another thread or two or three about issues with the stereo adapter and the controller.

    If you're saying you controller doesn't work at all when you plug in a headset adapter of any tpye then I suggested to others batteries (try that first) but if not batteries then its totally possible that you got a bad controller but I doubt everyone got
    a bad batch of controllers.

    Use Alkaline AA batteries or rechargeable batteries that have at least 1700 mAh

    Pretty sure its battery related.
