Xbox One controller disconnects all the time windows 10

I have a wired xbox one controller. Bought it about a month ago. The entire time it's been sitting on the computer, never dropped or anything. It's started disconnecting constantly. I'll be playing a game, it'll vibrate, and it will disconnect.

I tried chatting with someone, but when I asked the live chat bot to connect me to someone it said "you can connect with a live person below" and then just sat there and sate there.

Anyone have any ideas?
